DHU Healthcare is seeking a Care Navigator to join our Primary Care teams, providing essential front-line support to patients and clinicians across our Derbyshire practices.

Location: Eyam & Bradwell Surgeries, S32 5QH (with flexibility across Derbyshire practices)
Contract: 12-month fixed term contract
Hours: 0 hours
Competitive Salary: £12.21 per hour (Band 2)

What Your Typical Day Looks Like:
Acting as the first point of contact for patients, providing a warm and professional welcome
Navigating patients to the right care quickly and efficiently – whether that’s a GP, nurse, pharmacist, or other services
Managing appointment bookings and practice communications with accuracy and care
Supporting clinical teams by handling essential admin and digital systems
Providing clear information and signposting to local health and community services
Contributing to practice efficiency, patient satisfaction, and quality improvement initiatives
Working collaboratively with GPs, nurses, and other healthcare professionals
To Be a Successful Care Navigator You’ll Need:
Excellent communication and interpersonal skills
Strong organisational and multitasking abilities
A patient-focused, compassionate approach
Confidence using IT and digital systems
Ability to work well in a busy team environment
Commitment to confidentiality and safeguarding principles
Flexibility to support service and rota needs
