As a Practice Nurse you will sit at the heart of delivering safe, effective, and patient-centred care within the community. By combining clinical expertise with autonomous decision-making, the post holder ensures that patients receive timely, evidence-based treatment while maintaining dignity, compassion, and professionalism at all times. The role also plays a vital part in managing long-term conditions, preventing illness, and promoting healthier lifestyles, which ultimately improves overall population health. Beyond direct patient care, it contributes to the strength and sustainability of the service through leadership, mentorship, and continuous quality improvement, helping to develop both colleagues and the wider practice. In doing so, the role supports not only individual patient outcomes but also the efficiency, reputation, and future growth of the organisation.

As a Practice Nurse you will:
- Deliver safe, effective, and evidence-based nursing care autonomously within own scope of practice
- Manage and support patients with long-term conditions (e.g. asthma, diabetes, CHD) through clinics and ongoing care
- Maintain accurate clinical records and ensure compliance with professional, legal, and organisational standards
- Collaborate with the multidisciplinary team and contribute to service improvement, leadership, and mentoring of staff
